Required Paperwork

If LAX is the first sport you are playing this year you will need to fill out and return all the Player Forms linked on the right hand side of this page to be eligible to play.  RIIL WILL NOT ALLOW AN ATHLETE TO PLAY WITHOUT PROPER DOCUMENTATION!!

If you have played a sport this year for NHS, all you need to do is make sure your last physical was on or after 15 Jun 2011.  I have sent Emails to everyone in this situation.  Please return your physical to the school nurse.

I may be asking some athletes to provide a "Pre-Participation" form if I cannot locate the original you filled out.  This form provides authorization for medical treatment in the event of an emergency.  I need to have this form for all players and carry them with me at all practices / games.

All athletes must review and sign the 2012 Team Policies with their parents / guardians and return it to me ASAP!
